As a - Field Based -Community Health Worker (CHW) , you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. -CHW's work in a team-based structure and spend the majority of their time in the community engaging directly with members. -Interesting in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role.
This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am - 5:00pm. -It may be necessary, given the business need, to work occasional overtime. This position is a field-based position with a home-based office. You will work from home when not in the field.
Primary Responsibilities:
Responsible for: intake/screening/referral, assessment/reassessment, development of support plans, on-going case management, monitoring of the Member's health and welfare, documentation of contacts and case management activities in the Department-prescribed IMS, resource development, and case closure.
Respect confidentiality and maintain confidences as described in the UHG Employee Handbook and acknowledged through signature by all employees. Maintain confidentiality of patient information. The ability to maintain confidentiality is a critical and essential component of this position.
Contact the Member's at least once within each quarterly period, or more frequently if warranted by the Member's condition /or as determined by the rules of the LTSS Program in which the Member's is enrolled.
Conduct face-to-face contact with the Member's at least every six (6) months, or more frequently if warranted by the Member's condition or the rules of the LTSS Program in which the Member is enrolled, and shall update the current approved assessment tool and Support Plan in the appropriate documentation system to reflect any changes in condition or services.
Complete a new assessment during a face-to-face reassessment annually, or more frequently if warranted by the Member's condition or if required by the rules of the LTSS Program in which the Member is enrolled. Complete additional assessments as needed/required.
Monitor the delivery of services and supports identified within the Support Plan and the Prior Authorization Request (PAR). This includes monitoring: The quality of services and supports provided
The health and safety of the individual; and
The utilization of services with respect to the Support Plan and the PAR.
The following criteria may be used to determine the Member's level of need for case management services:
Availability of family, volunteer, or other support
Overall level of functioning
Mental status or cognitive functioning
Duration of disabilities
Whether the individual is in a crisis or acute situation
The individual's perception of need and dependency on services
The individual's move to a new housing alternative
Whether the individual was discharged from a hospital or Nursing Facility.

Attend Department provided training for SEP agencies as well as agency provided in-service and staff development training.
Responsible to conduct work that is in accordance with the SEP Contract Agreement's requirements. This includes but is not limited to, providing work that is accurate, timely, and complete.
May take on special assignments and projects on an as needed basis. Assist with system assessment and development as needed.
This position requires travel. Requires independent, reliable, flexible, and on-demand, transportation at the incumbent's expense for travel between various locations and timely arrival and departure from various locations. If the employee chooses to satisfy this requirement by driving a vehicle, the employee must meet the requirements for Colorado licensure and company requirements for liability insurance coverage.
Complete all duties in accordance with company safety policies and practices.
Other functions may be assigned and management retains the right to add or change the duties at any time.

Required Qualifications:
Bachelor's degree (or higher) in human behavioral science fields such as human services, nursing, social work, psychology, public health, OR 5+ -years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities OR 5+ years combined pertinent education and relevant experience
Required to work a flexible schedule including before and after core business hours and occasional evenings and weekends as business needs dictate
Required to travel up to 50% locally on a daily basis while being exposed to changing weather conditions
Reside within Mineral County, CO -or Rio Grande County, CO
Required to have a dedicated work area established that is separated from other living areas and provides information privacy
Ability to keep all company sensitive documents secure (if applicable)
Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Ability to work in front of a computer screen and keyboard, sitting and/or standing up to 8-10 hours per day (possibly longer if required).
Ability to work in home settings that include varying physical, social, and cultural environments.
Ability to work in environments that may have exposure to communicable disease while performing patient care activities.
Ability to see adequately to visually evaluate and assess the member's or potential member's physical condition and living environment.
Ability to see computer screen and work papers.
Ability to access, research and interpret information from a variety of media (e.g., computer screen, projected images, printed material) and individuals.)
Ability to work in normal office environment conditions and with various office equipment (e.g., computer, keyboard, mouse, calculator, copier, printer, fax, scanner, telephone).
Ability to move to access various offices and a wide variety of meeting settings.
Ability to perform a variety of tasks that involve standing, walking, sitting, stooping, kneeling, bending and twisting, occasionally climbing stairs or using an elevator, possibly reaching chest high and overhead for materials.
