Overview: The clinical pharmacy manager is responsible for
coordinating clinical pharmacy services and ensuring that the
pharmacy provides optimal clinical services on a daily basis. The
clinical pharmacy manager facilitates the development,
implementation, and maintenance of systems that promote desirable
patient outcomes through the use of safe, appropriate,
cost-effective medication therapies. The clinical pharmacy manager
will act on behalf of the Director of Pharmacy in their absence,
will manage day to day operations of the inpatient pharmacy, and
will assist in developing competencies, policies and procedures,
and managing medication errors.Primary Scope of
Responsibilities:
- Directs development, implementation, and maintenance of
clinical pharmacy services
- Facilitates the development and implementation of treatment
guidelines, protocols, formulary changes, and critical pathways as
approved by appropriate committees.
- Identifies and promotes new learning around changing patient
safety standards of practice.
- Coordinates the selection and safe use alternative medications
in response to medication shortages and provides timely
communication to appropriate providers and nursing departments
- Supervises and instructs pharmacists, pharmacy technicians, and
students
- Prepares and presents all required agenda items for
P&T
- Contributes to the quality and effective operation of the
department by reviewing medication events and following up with
appropriate staff.
- Interprets medication orders (verbal and written) and
transcribes to computerized patient medication profiles accurately.
Determines accuracy of medications by reviewing medication therapy
regimens for drug-drug interactions, allergies, incompatibilities,
appropriateness of drug and dose and therapeutic duplications.
- Prepares and dispenses sterile and chemotherapy compounding as
required using aseptic technique and ensuring compatibility and
stability of all added ingredients. Labels all preparations with
appropriate data and warnings. Also compounds special solutions,
ointments, etc. when not commercially available. Dispenses
medications accurately ensures that they are safe, appropriate,
cost-effective medication therapies for patients.
- Assists with pharmacokinetics consult service, renal dosing
services, IV to PO switch services, antimicrobial stewardship
services, and any other therapeutic drug management per
organizational protocols
- Maintains records pertaining to all phases of drug and supply
ordering, storage, and distribution as required by hospital policy
and/or governmental regulations. Completes necessary paperwork for
the preparation of patient charges, pharmacy statistical data,
scheduled drug control records, patient profiles, etc. Ensures all
equipment, balances, Laminar air flow hood, etc. are maintained in
good working order and that work area is maintained in an orderly,
clean and when necessary aseptic manner. Ensures nursing stations
are periodically checked for outdated and overstocked
medications.
- Clinical pharmacists identify, resolve, and prevent
medication-related problems. They recommend and monitor patient
medication therapy and provide drug information. Clinical
pharmacists interact with patients, family, and other healthcare
providers as appropriate to ensure continuity of services and to
achieve desired patient outcomes through safe, timely,
cost-effective medication therapy.
- Assumes daily procedural responsibilities of the Director of
Pharmacy in their absence or an emergency situationMinimum
Qualifications & Requirements:
- Graduation from an accredited school of pharmacy and registered
with the State of Colorado to practice pharmacy
- Knowledge of procedures, schedules and basic medical
terminology
- Current State of Colorado pharmacy license
- Current BLS certification within one year of employment; ACLS
