per week About The Position Job Title: Hospice Registered Nurse
Profession: Nursing Specialty: Hospice RN Duration: 13 weeks Shift:
Days Hours per Shift: 32 Experience: Minimum of 3 years of nursing
experience, preferably in hospice, home health, or palliative care
License: Current Registered Nurse (RN) license in the state of Utah
or compact state RN license Certifications: Current BLS
Certification Must-Have: Strong clinical assessment skills
Excellent communication, interpersonal, and organizational skills
Compassionate demeanor and ability to support patients and families
during difficult times Valid driver's license and reliable
transportation required for home visits Description: We are seeking
a dedicated and compassionate Hospice Registered Nurse (RN) to join
our interdisciplinary team. The RN will provide skilled nursing
care and support to terminally ill patients and their families in
accordance with the patient's plan of care and hospice philosophy.
Perform comprehensive assessments of patients' needs and coordinate
individualized care plans. Provide pain and symptom management
consistent with the hospice plan of care. Educate and support
patients and families regarding the disease process, self-care
techniques, end-of-life issues, and the hospice philosophy.
Collaborate with the interdisciplinary team including physicians,
social workers, chaplains, and volunteers. Participate in on-call
rotation. Maintain accurate and timely documentation in compliance
with state, federal, and organizational policies. Able to respond
within 45 minutes to patient needs when on call. Able to work
holidays. Experience with hospice EMR systems preferred.
