Summary: The Physical Therapist is responsible for evaluating
residents, developing personalized treatment plans, and delivering
physical therapy services in a skilled nursing facility. The PT
works to improve residents’ strength, mobility, balance, and
overall physical function, with the goal of enhancing independence
and quality of life. All services must comply with federal, state
(Colorado), and facility regulations. ABOUT US –Mantey Heights is

Essential Duties and Responsibilities: - Perform initial and
ongoing assessments of resident physical function and mobility. -
Develop, implement, and revise individualized treatment plans based
on assessment results. - Administer physical therapy treatments and
interventions in line with best practices. - Educate residents,
families, and staff on proper techniques and home exercise
programs. - Document evaluations, treatment sessions, progress
notes, and discharge summaries accurately and timely. - Collaborate
with the interdisciplinary care team, attending care conferences
and assisting with discharge planning. - Supervise and delegate
duties to Physical Therapist Assistants (PTAs) as required by
Colorado licensure laws. - Ensure compliance with Medicare,
Medicaid, and facility documentation and billing standards. -
Maintain equipment and work areas to ensure safety and cleanliness.
Qualifications: - Doctorate or Master’s degree in Physical Therapy
from an accredited institution. - CurrentColorado Physical
Therapist Licensein good standing. - Valid CPR certification. -
Previous experience in skilled nursing, long-term care, or
geriatric rehab preferred. - Strong interpersonal, documentation,
and organizational skills. - Ability to work effectively in a
team-oriented, resident-centered environment. Physical
Requirements: - Must be able to lift up to 50 pounds. - Frequent
standing, walking, reaching, and assisting with resident transfers
or ambulation. - Ability to perform tasks that require physical
stamina and mobility in a clinical setting. Compensation and
Benefits: - Competitive salary $40/hr - $50/hr DOE - Medical,
