Physical Therapist Location: Grand Junction, CO Agency: Acruex Health Pay: $2,080 to $2,240 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Pay Rate: $52.00 - $56.00/Hourly. Summary: We are looking for a licensed Physical Therapist to join our team of health professionals in providing excellent health care in our facility. Typically, you'll be working with post-op patients to help them with recovery. You will be consulting with them to learn about their current condition and assess their patient evaluations in order to develop proper treatment plans. Responsibilities: Perform assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits. Develop and implement individualized physical therapy plans of care based on patient needs and physician orders. Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention. Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies. Communicate patient progress, concerns, or changes in condition to the agency’s interdisciplinary care team. Requirements Requirements: Current, unrestricted Colorado Physical Therapist license.
