Occupational Therapist Location: Grand Junction, CO Agency: Windsor Healthcare Recruitment Group, Inc. Pay: $2,431 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position We are seeking a licensed Occupational Therapist (OT) to provide skilled, in-home occupational therapy services to patients in the Grand Junction area. The Occupational Therapist delivers patient-centered, evidence-based occupational therapy services in patients homes in accordance with physician orders, payer requirements, agency policies, and applicable state and federal regulations. This role supports patient safety, functional independence, and quality of life through individualized care focused on activities of daily living and adaptive strategies. Clinical Responsibilities Perform assigned occupational therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits Develop and implement individualized occupational therapy plans of care based on patient needs, functional goals, and physician orders Provide skilled interventions such as ADL and IADL training, upper extremity strengthening, fine motor coordination, adaptive equipment training, energy conservation, and home safety and fall-prevention strategies Educate patients and caregivers on treatment plans, adaptive techniques, home programs, and safety strategies Communicate patient progress, concerns, and changes in condition to the agency s interdisciplinary care team Collaborate in the clinical oversight of Occupational Therapy Assistants (OTAs), when applicable and permitted under Colorado law and agency policy Documentation & Systems Complete all required documentation accurately and timely in Homecare Homebase (HCHB) Ensure documentation complies with Medicare Conditions of Participation, payer requirements, and agency standards Maintain compliance with HIPAA and all applicable privacy and security requirements Care Coordination Collaborate with agency clinical staff, including RNs, OTAs (when applicable), PTs, STs, and leadership Participate in care coordination related to patient needs, visit completion, and regulatory requirements Provide OTA oversight only when applicable, assigned, and permitted under Colorado law and agency policy Qualifications Required Current, unrestricted Colorado Occupational Therapist license Graduation from an accredited Occupational Therapy program Valid driver s license and reliable transportation Ability to practice independently in a home-based clinical setting Proficiency with, or willingness to learn, Homecare Homebase (HCHB) Preferred Home health experience Familiarity with Medicare documentation and home health regulations Experience working with geriatric and medically complex patient populations Practice Environment Patient-centered, in-home care One-on-one visits Supportive clinical and administrative coordination Mission-driven home health services serving the Grand Junction community Shift: Monday-Friday 8 am-5 pm Specialty Type: Rehabilitation Therapy Sub Specialties: Occupational Therapist General Certifications: N/A Please CLICK HERE to view details.
