ESSENTIAL FUNCTIONS I

CLINICAL PROCESS & PRACTICE SKILLS Fully competent in primary role on unit; performs patient and family assessment and problem identification, plans care accordingly, implements and evaluates plan of care so that problems are resolved. Adheres reliably to all accepted patient safety standards. Identifies and promotes new learning around changing patient safety standards of practice. Serves in advocacy role; encouraging active involvement of patient and family in their care. Prioritizes patient's situations and takes appropriate action in an efficient, effective manner. Makes value-added resource decision for patient care with respect to supplies, equipment and acuity of care. II

PATIENT TEACHING Keeps patients informed regarding what to expect pre/during/post procedure Provides individualized patient and family education consistent with patient's learning style, readiness to learn and self care needs Evaluated the effectiveness of patient learning III

COMPLIANCE WITH RULES, REGULATIONS AND POLICIES Records pertinent clinical documentation in accordance with regulatory requirements, professional standards and hospital policy. Adheres to federal, state and hospital rules and regulations concerning HIPAA privacy and security. Follows Hospital compliance plan and policies, including the Code of Conduct. Participates in required training for compliance related activities (HIPAA, Compliance, Patient Safety, TJC, etc.) IV

Develops professional goals and diversity of responsibilities within practice; consistent with hospital mission and goals Demonstrates role awareness and seeks individualized/department specific continuing education that improves patient care or professional practice. Able to express values around patient/family centered care and assists others to achieve shared understanding. Active participant in quality improvement processes. Participates and contributes to unit meetings and activities. Identifies need to seek out additional information when needed and apply relevant findings to practice V

SERVICE EXCELLENCE & CUSTOMER SERVICE Demonstrates purposeful service excellence through exceeding patient and co-worker expectations. Represents the department and hospital as a dedicated, courteous and responsive employee. VI UNIT SPECIFIC DUTIES & RESPONSIBILITIES Develops annual budget reflecting needs and priorities. Maintains appropriate resources for effective and creative teaching environment including:

a. Privacy

b. Material and teaching aids to meet individual needs

c. Adequate number of training staff Monitors quality through CQI programs; reports to advisory committee annually Maintains the quality diabetes education center and follows the mission and vision statement and appropriate guidelines and manuals. Records diabetes education and maintains all documentation needed for third party payers and ADA standards of care, up to and including calling prescriptions in for diabetes supplies, pre-certification of benefits and writing appeal letters to insurance companies that deny education benefits. Teach and meet the needs of the patients in group or individual sessions including all of the following subjects:

a) Diabetes overview/pathophysiology of diabetes,

b) Nutrition and food planning,

c) Exercise and activity,

d) Diabetes medication (including skills related to the self-administration of injectable drugs),

e) Self-monitoring and use of the results,

f) Prevention, detection and treatment of acute and/or chronic complications,

g) Foot, skin, and dental care,

h) Behavior change strategies, goal setting, risk factor reduction and problem solving,

i) Preconception care, pregnancy, and gestational diabetes,

j) Relationship among nutrition, exercise, medication, and blood glucose levels,

k) Stress and psycho social adjustment,

l) Family involvement and social support,

m) Benefits, risks, and management options for improving glucose control,

n) Use of health care systems and community resources. When receiving a consult from MD or RN will provided for in-patients with survival skills only and will refer education on an out-patient basis. For out-patients the education will be interactive, collaborative and an ongoing process that involves the person with diabetes and the educators; acts as a coach including calling out-patient between visits. Collaborates with the multi-disciplinary team of healthcare professional to help coordinate health education of the diabetes patients; reports progress/outcome from educational experience to the referring primary care provider; serves as a resource for continuity. Helps collect patient satisfaction through written surveys after each class/session. Pursue professional education through advance curriculum; maintains the requirements for CDE eligibility; promote healthy behavior by role modeling self-care techniques. Help with diabetes training to staff nurses or other healthcare professional by lecture or competency module as required. POPULATION SPECIFIC CRITERIA
